Vendor Registration

Vendors are requested to complete a Vendor Registration Form. This registration form includes a Commodity list so that your company may be included in upcoming opportunities. Please indicate the commodity/commodities that your company provides by checking the box next to the commodity. Select only the commodities that apply to your company.

Each vendor is responsible for providing updates to the Town for any changes to their information in order to maintain their listing.

Please follow these instructions when submitting your registration:

  1. Complete the Vendor Registration Form.
  2. Select the commodity/commodities supplied by checking the appropriate box(s).
  3. Sign the completed form.

Vendor Registration Form

IRS Form W-9

The W-9 form is required for all new registrations and all changes to existing vendor registrations. In order to fulfill IRS requirements, we need to have a W-9 form for each company with whom we do business. Please download/print this form and submit it to the Town.

Tax Exemption Certificate

The Town of Palm Beach is exempt from the payment of Florida sales tax.
